Attention Parents:

If your student has medication(s) in the Nurse's office, it must be picked up by a parent by the last day of school- Friday, June 7, 2013.

No medication can be left on campus over the summer.

Medications that are not picked up by June 10, 2013 at 12:00pm, will be distroyed per the medication policy.

Please pick up any forms you may need for next year's medications when you are here.  There must be a new form filled out by a physician for each school year.
