  • Athletic Participation Form
  • Insurance Waiver Form
  • Domicile (to be completed once during high school career unless there is a change of address)
  • Physical (Due once per calendar year)
  • Participation Requirements Form
  • Assumption of Risk Form
  • Student-Athlete Contract
  • Gfeller-Waller Concussion Statement

Honoring your contribution to Mount Pleasant High School. These 18” x 24” banners will hang year-round on our gymnasium’s “Legacy Wall” to show your graduation year and the Varsity letters you earned while participating in our athletic program at Mt. Pleasant High School. They will also recognize the contributions of our administrators, coaches and teachers though out the years.  There is a one-time charge of $60 for each individual banner order placed.
