Registration Information

Registration Information Needed
Please provide the following information when registering a student.

PROOF OF DOMICILE - TWO ARE REQUIRED Deed, Lease, or Tax Statement showing that property is owned or rented by the parent or guardian AND Utility bill, or bank statement, or current drivers license showing the correct name and address of student's parents

PROOF OF GUARDIANSHIP - When child is residing with someone other than parent listed on Birth Certificate. Legal court documents only.

BIRTH CERTIFICATE - Copy of Birth Certificate (Certified copies only)
 
IMMUNIZATION RECORD (SHOTS) - Copy of current immunization.
Parents: Please be aware that a Picture ID will be required when enrolling a child and that you will be asked to sign a notarized form.

PREVIOUS SCHOOL RECORDS - During summer registration please bring a report card from the end of the previous school year. 

During the school year please bring a withdrawal from your previous school.

All sixth grade students for the 2011-2012 school year will be required to have one (1) booster of Tdap (tetanus/diphtheria/pertussis) vaccine, if five (5) years or more have passed since the last DTP/Td (tetanus/diphtheria).
